statement on your company letterhead that references our purchase order number,our code and states the date when and place where this
product was manufactured.This document will then need to accompany your shipping documents.This will be a requirement for submission for payment
regardless of the terms of your order.We will not be able to pay you for the shipment unless we receive this document.This applies to all adult or children’s
items.

Proposition 65 Certification:Please advise if the product will be manufactured using a chemical(s) known to the state of California to cause cancer or
reproductive toxicity,or the level of such chemical(s) present in the product does not exceed or is not emitted upon use of such product at a level greater than
the level specified as having no significant rise by the California Code of Regulations,Title 22,Chapter 3,Article 7.
